Tampa Family Health Centers Brings Rheumatology Care to South Tampa with Opening of Swann Location

Tampa Family Health Centers (TFHC) celebrated the grand opening of TFHC Swann, located at 2111 Swann Ave. Suite 102, officially bringing Rheumatology services into TFHC for the first time and expanding access for patients managing autoimmune and inflammatory conditions.

The opening also marked the addition of rheumatologist Dr. Carolyn Connelly to the TFHC network, a Tampa native board certified in Rheumatology and Internal Medicine with more than 37 years of experience caring for patients throughout the Tampa Bay area. Patients continue to receive the trusted care they know from Dr. Connelly, now supported by a more connected and coordinated care experience.

“For years, I’ve had the privilege of caring for my patients and being part of their health journey,” said Dr. Connelly. “That relationship remains the same. What’s new is the level of support now surrounding each patient, making care more connected and easier to manage.”

TFHC Swann reflects the organization’s continued investment in expanding access to specialty services while maintaining a patient centered approach. The practice combines expert Rheumatology care with support from an onsite clinical pharmacist, helping create a more seamless experience for patients navigating diagnosis, treatment, and ongoing management.
