A Patient Centered Medical/Dental Home is called a “home” because we would like it to be the first place you think of for all your healthcare needs. Much like a home, our goal is to make it easy and comfortable for you to get the care you need in a way that works best for you. Within our home, a relationship develops between you and your Care Team: the providers, clinical support, and office staff dedicated to coordinating the services you need for the best care possible. The focus is on you.

 

Your Primary Care Provider Pledges To

  • Coordinate your care across all settings (for example specialists, rehabilitation centers, behavioral health).
  • Be available during and after regular business hours for your care needs.
  • Explain diseases, treatments, and results in an easy-to-understand way.
  • Learn all about you, your history, your care preferences, and your health goals.
  • Keep treatments, discussions, and records private.
  • Provide instructions on how to meet your healthcare needs after hours.
  • Give you clear directions about treatments and medications.
  • Listen to your questions and concerns to help make decisions about your care.
  • Provide optimal care based on nationally researched clinical protocols.
  • Communicate in a timely fashion; test results, information about alternative services to help you maintain a healthy lifestyle, and updates on vaccines and screenings.
  • End every visit with clear instructions about expectations, treatments, and future plans.

How Do I Become a Patient?

Becoming a patient at Tampa Family Health Centers is easy, and anyone can join. Start taking advantage of our affordable health services and the convenience offered with 16 locations throughout Hillsborough County, Florida.

Request Appointment

Schedule your first appointment by calling us at 813-397-5300 or using our online scheduling.


STEP 2

Patient Intake Forms

Complete our Patient Intake form, or it can be completed in person on the day of your scheduled appointment.


STEP 3

Visit a Provider

After completing your Patient Intake Forms, the only thing left to do is attend your scheduled appointment.

How Do I Pay for Services?

We accept most types of insurance plans, including Medicare, Medicaid, HMOs, PPOs, KidCare, Medikids, Healthy Kids, Children’s Medical Services (CMS), and other plans. Certified Application Counselors are available to assist in finding and enrolling in insurance options through the marketplace.

  • Payment is expected on the day services are provided.
  • Co-pays or the nominal fees are collected at check-in. At check-out, additional fees may be due based on the services you received.
  • You are encouraged to apply for our sliding scale program. If eligible, the program may reduce costs for the uninsured or be applied to insurance deductibles and co-insurances.
  • We serve all patients regardless of their ability to pay.

Items to Bring to Your First Visit

  1. Photo identification
  2. Health insurance card(s)
  3. Social Security card
  4. Proof of income for Sliding Discount Program
  5. Medical or dental history
  6. Medical Records – especially if you’ve recently been admitted to the hospital or ER
  7. All medications currently taking
  8. Foster parent or grandparent: Guardian or custody papers showing custody of child required
  9. Adult guardian: Power of Attorney

Access Availability

We offer same-day access at all 16 locations. Contact us at (813) 397-5300 to make an appointment or walk-in to your participating center. For faster service please bring a photo ID, insurance card (if applicable), copay, list of medications you’re currently taking, and any medical records that you may have.
 

After Hours

Providers are available for after-hours emergency calls through our answering service at 813-397-5300. In the event of an emergency, please call 911.

FAQs

Are services free?

No, services at the Tampa Family Health Centers (TFHC) are not free. Patients pay for services according to their income and household size by applying for our sliding fee schedule.

What do I do if I can’t pay for my care?

Your ability to pay should not stop you from receiving the health care you need. You might be eligible to set up a payment plan or we can arrange for you to pay for services on a sliding fee scale.

How can I obtain HCHCP or Medicaid?

You can see a patient resource specialist at one of our Centers and they can assist you with the application process.

Can anyone apply for the sliding fee schedule?

Yes. We encourage everyone to apply for the sliding fee schedule.

Can I come to TFHC if I have insurance?

Yes. We accept a large number of health insurance plans including Medicaid and Medicare. Be sure to bring your insurance card with you for any visits at TFHC.

If I have insurance with a high deductible or uncovered services, can I apply for the sliding fee schedule?

Yes. Insured patients may also be eligible for discounts for uncovered insurance services and the sliding fee schedule based on income and family size.

What forms of payment do you accept?

Cash, check, and major credit cards.

Do I need to make an appointment?

We encourage you to make an appointment. However, we will accept walk-in patients as our schedule permits.
